Chronic kidney disease in immune thrombocytopenia (ITP) is uncommon, and renal transplant in this setting is rare. We discuss the successful renal transplant of a 29-year-old male with chronic ITP. During transplant, he was managed with thrombopoietin receptor agonist eltrombopag, intravenous methylprednisone, and intravenous immunoglobulin to maintain adequate platelet level. Tacrolimus is the most important drug in current posttransplant immunosuppressive protocol. Salt-losing nephropathy causing symptomatic hyponatremia as an adverse effect of tacrolimus has been rarely reported. We report recurrent hyponatremia and graft dysfunction in a young renal transplant recipient, with no evidence of rejection, attributable to. tacrolimus-induced salt-wasting nephropathy.
