OBJECTIVE: Coding of obesity using the International Classification of Diseases (ICD) in healthcare administrative databases is under-reported and thus unreliable for measuring prevalence or incidence. This study aimed to develop and test a rule-based algorithm for automating the detection and severity of obesity using height and weight collected in several sections of the Electronic Medical Records (EMRs). METHODS: In this cross-sectional study, 1904 inpatient charts randomly selected in three hospitals in Calgary, Canada between January and June 2015 were reviewed and linked with AllScripts Sunrise Clinical Manager EMRs...

UNLABELLED: Sepsis is a worldwide public health problem. Rapid identification is associated with improved patient outcomes-if followed by timely appropriate treatment. OBJECTIVES: Describe digital sepsis alerts (DSAs) in use in English National Health Service (NHS) acute hospitals. METHODS: A Freedom of Information request surveyed acute NHS Trusts on their adoption of electronic patient records (EPRs) and DSAs. RESULTS: Of the 99 Trusts that responded, 84 had an EPR...

OBJECTIVES: The use of electronic health record (EHR)-embedded child abuse clinical decision support (CA-CDS) may help decrease morbidity from child maltreatment. We previously reported on the development of CA-CDS in Epic and Allscripts. The objective of this study was to implement CA-CDS into Epic and Allscripts and determine its effects on identification, evaluation, and reporting of suspected child maltreatment. MATERIALS AND METHODS: After a preimplementation period, CA-CDS was implemented at University of Wisconsin (Epic) and Northwell Health (Allscripts)...

BACKGROUND: A growing body of literature has linked usability limitations within electronic health records (EHRs) to adverse outcomes which may in turn affect EHR system transitions. NewYork-Presbyterian Hospital (NYP), Columbia University College of Physicians and Surgeons (CU) and Weill Cornell Medical College (WC) are a tripartite organization with large academic medical centers that initiated a phased transition of their EHRs to one system, EpicCare©. OBJECTIVES: To characterize usability perceptions stratified by provider roles by surveying WC ambulatory clinical staff already utilizing EpicCare© and CU ambulatory clinical staff utilizing iterations of Allscripts© before the implementation of EpicCare© campus-wide...

BACKGROUND: Electronic health records (EHRs) have been connected to excessive workload and physician burnout. Little is known about variation in physician experience with different EHRs, however. OBJECTIVE: To analyze variation in reported usability and satisfaction across EHRs. DESIGN: Internet-based survey available between December 2021 and October 2022 integrated into American Board of Family Medicine (ABFM) certification process. We interviewed six clinicians to learn about their lived experience using electronic health records (EHR, Allscripts users) using a semi-structured interview guide in an academic medical center in New York City from October to November 2016. Each participant interview lasted approximately one to two hours. We applied a clustering algorithm to the interview transcript to detect topics, applying natural language processing (NLP). We visualized eight themes using network diagrams (Louvain modularity 0.70). Novel findings include the need for a concise and organized display and data entry page, the user controlling functions for orders, medications, radiology reports, and missing signals of indentation or filtering functions in the order page and lab results...

BACKGROUND: Routine collection of disease activity (DA) and patient-reported outcomes (PROs) in rheumatoid arthritis (RA) are nationally endorsed quality measures and critical components of a treat-to-target approach. However, little is known about the role electronic health record (EHR) systems play in facilitating performance on these measures. OBJECTIVE: Using the American College Rheumatology's (ACR's) RISE registry, we analyzed the relationship between EHR system and performance on DA and functional status (FS) quality measures...

PURPOSE: Since the beginning of the coronavirus disease 2019 pandemic in early March, there has been a push to expand virtual patient care visits instead of in-person clinic visits. Studies have found that telemedicine can provide efficient triaging, reduction in emergency room visits, and conservation of health care resources and personnel. Although virtual patient care has been implicated in providing similar outcomes to traditional face-to-face care in patients affected with coronavirus disease 2019, there are a lack of studies on the effectiveness of virtual care visits (VCVs) for patients with craniosynostosis or deformational plagiocephaly...

METHODS: Winpath histology database from January 2011 to April 2018 identified all suspected IgG4RD cases wherein IgG4 immunohistochemistry was performed. The histology slides were reviewed to categorise cases into Boston criteria groups-highly suggestive of IgG4RD, probable IgG4RD and insufficient evidence. QUESTION ASKED: What is the most important information that diverse institutional stakeholders at a comprehensive cancer center need to know about patients to provide patient-centered care, and what is the best way to display this information in a new single-location feature in the electronic health record (EHR)? SUMMARY ANSWER: Thematic content analysis of semistructured interviews with a large and diverse group of institutional stakeholders at our comprehensive cancer center revealed themes informing design and development of the Patient Values Tab EHR feature, generated enthusiasm and buy-in for this digital innovation, created a sense of awareness among future users, and paved the way for implementation...
