For decades, digoxin has been widely used to control ventricular rate in atrial fibrillation (AF). However, it remains controversial as to whether digoxin is associated with increased mortality in AF. In this study, we searched relevant studies that were published before December 1, 2014, in PubMed, EMBASE, and the Cochrane central databases. We systematically reviewed the references and performed a meta-analysis of 8 carefully selected studies with 302,738 patients who were included for the final analysis...

INTRODUCTION: The inodilator levosimendan was developed as a treatment for acutely decompensated severe chronic heart failure. In recent years, its use has broadened to treatment of heart failure in different settings. These include advanced chronic heart failure, and other scenarios where haemodynamic stability is sought, such as pre-operative treatment of patients at risk of low cardiac output syndrome or peri-operative heart failure. The aims of this presentation of four case reports were to compare the use of levosimendan in different settings, and to highlight differences and similarities in the effects obtained, with the purpose of defining common guidance on the use of levosimendan...

OBJECTIVES: The purpose of this study was to compare the safety and efficacy of acetylsalicylic acid (ASA) and warfarin for thromboprophylaxis after the Fontan procedure. BACKGROUND: Fontan surgery is the definitive palliation for children with single-ventricle physiology. Thrombosis is an important complication; the optimal thromboprophylaxis strategy has not been determined. METHODS: We performed a multicenter international randomized trial of primary prophylactic anticoagulation after Fontan surgery...

BACKGROUND: Angiotensin-converting enzyme inhibitor therapy improves clinical outcome and ventricular function in adults with heart failure. Infants with single-ventricle physiology have poor growth and are at risk for abnormalities in ventricular systolic and diastolic function. The ability of angiotensin-converting enzyme inhibitor therapy to preserve ventricular function and improve somatic growth and outcomes in these infants is unknown. METHODS AND RESULTS: The Pediatric Heart Network conducted a double-blind trial involving 230 infants with single-ventricle physiology randomized to receive enalapril (target dose 0...
