Annually, about 5-10% cases of newly diagnosed rectal cancer are locally advanced (T4). An R-0 resection is the single most predictor of outcome. This has been made possible by surgical resection beyond the regular TME (Total Mesorectal Excision) plane, also known as extended rectal resection which involves removal of the total mesorectum along with en bloc resection of involved pelvic structures.
