BACKGROUND: The three-step Brugada group algorithm is the only published electrocardiographical (ECG) algorithm for differentiating ventricular tachycardia (VT) from pre-excited tachycardia (PXT) as a cause of regular wide QRS complex tachycardia (WCT). This study aimed to improve the diagnostic accuracy of the Brugada group algorithm. METHODS: This study modified the Brugada group algorithm by adding a new aVR lead criterion (initial positive deflection in lead aVR and the QRS complex area above the baseline is greater than the area below the baseline)...

BACKGROUND: Artificial intelligence-augmented ECG (AI-ECG) refers to the application of novel AI solutions for complex ECG interpretation tasks. A broad variety of AI-ECG approaches exist, each having differing advantages and limitations relating to their creation and application. PURPOSE: To provide illustrative comparison of two general AI-ECG modeling approaches: machine learning (ML) and deep learning (DL). METHOD COMPARISON: Two AI-ECG algorithms were developed to carry out two separate tasks using ML and DL, respectively...

Irregular narrow QRS complex tachycardia is associated with a wide range of differential diagnoses, including atrial fibrillation and atrial tachyarrhythmia with altered atrioventricular conduction. Here, we present a case of narrow QRS complex tachycardia with variable R-R intervals and discrete P waves. ( Level of Difficulty: Intermediate. ).

A 19-year-old boy with rheumatic heart disease, post mitral valve replacement three years earlier, presented with recurrent episodes of palpitations. Sinus rhythm ECG showed left bundle branch block (LBBB). During symptoms, ECG showed regular wide complex tachycardia (WCT) at a rate of 140 beats per min (bpm) with QRS identical to sinus rhythm. Echocardiography revealed a dilated left ventricle with severely depressed systolic function. An electrophysiology study was performed. Four catheters - high right atrium, coronary sinus, right ventricle (RV), and multipolar catheter with close electrode spacing in the His region - were placed...

INTRODUCTION: A 13 old girl presented with recurrent wide QRS tachycardia since she was 4. METHODS: An electrophysiologic study was performed. RESULTS: The electrophysiologic study showed that QRS complexes identical to those of the tachycardia could be elicited with premature atrial extrastimuli but with a shorter atrioventricular (AV) delay when the QRS was wide compared with narrow QRS complexes. The tachycardia was ablated at 9 o'clock on the tricuspid annulus demonstrating the presence of an atriofascicular fiber...

AIMS: Fascicular ventricle tachycardia (FVT) arising from the proximal aspect of left His-Purkinje system (HPS) has not been specially addressed. Current study was to investigate its clinical, electrocardiographic, and electrophysiological characteristics. METHODS AND RESULTS: Eighteen patients who were identified as this rare FVT were consecutively enrolled, and their scalar electrocardiogram and electrophysiological data were collected and analysed. The ventricular tachycardia (VT) morphology was similar to sinus rhythm (SR) in eight patients, left bundle branch block type in one patient, right bundle branch block type in seven patients, and both narrow and wide QRS type in two patients...

Studies conducted during the last 50 years have proposed electrocardiographic criteria and algorithms to determine if a wide QRS tachycardia is ventricular or supraventricular in origin. Sustained ventricular tachycardia is an uncommon reason for consultation in the emergency room. The latter and the complexity of available electrocardiographic diagnostic criteria and algorithms result in frequent misdiagnoses. Good hemodynamic tolerance of tachycardia in the supine position does not exclude its ventricular origin...

BACKGROUND: Accurate automated wide QRS complex tachycardia (WCT) differentiation into ventricular tachycardia (VT) and supraventricular wide complex tachycardia (SWCT) can be accomplished using calculations derived from computerized electrocardiogram (ECG) data of paired WCT and baseline ECGs. OBJECTIVE: Develop and trial novel WCT differentiation approaches for patients with and without a corresponding baseline ECG. METHODS: We developed and trialed WCT differentiation models comprised of novel and previously described parameters derived from WCT and baseline ECG data...

Wide complex tachycardias (WCT) are frequently encountered in the emergency department. Causes of WCT vary from benign (eg, supraventricular with rate-related aberrancy) to life threatening (eg, atrial fibrillation with WPW, or ventricular tachycardia). It is imperative that emergency physicians are familiar with the clinical presentation, underlying causes, and electrocardiographic features of the different causes of WCT. Treatment of WCT depends on patient stability, regularity of the rhythm, and QRS morphology...

Lacosamide is an antiepileptic drug that acts on voltage-gated sodium channels and was approved as an antiepileptic by the Food and Drug Administration in 2008. Although the efficacy and safety of lacosamide have been established in many previous trials, some case reports have shown that it may lead to cardiovascular side effects, especially in patients with electrical conduction system disorders. We herein report a case of life-threatening cardiac arrhythmia caused by lacosamide intoxication that was successfully treated with veno-arterial extracorporeal membrane oxygenation...

A 44-year-old male patient presented to our clinic with nocturnal palpitations. The smartwatch electrocardiogram (ECG) demonstrated ST-segment deviation and non-sustained ventricular tachycardias. The patient suffered from coronary one-vessel disease. The coronary angiography revealed de novo proximal left anterior descending stenosis and in-stent restenosis, which necessitated coronary stenting. However, the recurrent palpitations reappeared 2 weeks later. The smartwatch ECGs again demonstrated ST-segment deviation and wide QRS tachycardia, reassessments with coronary angiography and magnetic resonance imaging revealed no relevant ischemia...

Anamorelin is prescribed for cancer cachexia treatment. Anamorelin is a ghrelin receptor antagonist and exerts a sodium channel blockade effect, possibly inducing disorders of the cardiac conduction system. We herein report two cases of wide QRS complex tachycardia caused by anamorelin. In both cases, the patients had liver dysfunction. Anamorelin is mainly metabolized in the liver; hence, sodium channel blockade by anamorelin during liver dysfunction can cause serious side effects, including wide QRS complex tachycardia, similar to flecainide toxicity...
