Angiomyofibroblastoma is a rare benign soft tissue tumor that arises predominantly in the female genital tract. It occurs less commonly in the scrotum, spermatic cord, and the retroperitoneum. Its diagnosis is based on specific histological and immunopathological features. However, the condition has never been previously described in the extremities. We present the first case report of angiomyofibroblastoma presenting as a slow-growing tumor in the foot of a 48-year-old lady.

Angiomyofibroblastoma is a rare, well circumscribed benign mesenchymal neoplasm, usually arising in the pelviperineal region, especially the vulva and apparently composed of stromal cells distinctive to this anatomic region. Preoperative diagnosis of Angiomyofibroblastoma is very difficult, mostly diagnosed as Barthloin cyst due to its anatomical location. It is a histological diagnosis. Angiomyofibroblastoma is a rare and benign tumor that usually involves vulvovaginal area in women of reproductive age and early menopause. We report a lipomatous angiomyofibroblastoma in a 55-year-old multigravid woman. Angiomyofibroblastoma is a very rare benign mesenchymal tumor of the superficial soft-tissues. Most of the cases have been reported in the vulva. It presents usually in the reproductive age group. Histologically it is characterized by alternating hypo and hypercellular areas admixed with thin walled blood vessels with a perivascular concentration of stromal cells. We report a case of this lesion arising in the vagina in a 46-year-old woman.

We report a rare case of angiomyofibroblastoma (AMFB) of the vulva, composed predominantly of a mature fatty component, representing approximately 60% of the entire tumour. The tumour, designated as "lipomatous AMFB", should be interpreted as the morphological result of an unbalanced bidirectional differentiation of the presumptive precursor stromal cell resident in the hormonally-responsive stroma of the lower genital tract, with the adipocytic component overwhelming the fibroblastic/myofibroblastic one. The close admixture of adipocytes with spindled/epithelioid cells of the conventional AMFB resulted, focally, in a pseudo-infiltrative growth pattern and pseudo-lipoblast-like appearance, raising problems in differential diagnosis, especially with well-differentiated lipoma-like liposarcoma and spindle cell liposarcoma...
