Mesenchymal tumours of the vulva are rare and consist of two types, difficult to distinguish but with different prognoses. Angiomyofibroblastoma (AMFB) is a benign tumour, whereas Aggressive Angiomyxoma (AA) is an infiltrating tumour. We describe a 22-year-old nulliparous patient with a vulvar mass sized 19 cm in diameter. After preoperative assessment by ultrasound, chest X-ray, and MRI, wide excision on the tumour was done and diagnosed as AMFB. Differentiation from AA is being discussed.

Mesenchymal lesions of the vulva include site-specific entities limited to the lower genital tract, as well as a range of non-site-specific tumors that are more common at extragenital sites. Site-specific lesions include fibroepithelial stromal polyp, cellular angiofibroma, angiomyofibroblastoma, and aggressive angiomyxoma. Non-site-specific tumors that may occur in the vulva include those of smooth muscle, skeletal muscle, vascular, neural, adipocytic, and uncertain differentiation. This review discusses both site-specific and non-site-specific vulvar mesenchymal lesions including non-neoplastic proliferations, benign neoplasms, locally aggressive neoplasms with a predilection for local recurrence, neoplasms of indeterminate biologic potential, and frankly malignant neoplasms with a high risk of distant metastasis and death...

Aggressive angiomyxoma (AA) is a rare mesenchymal neoplasm occurring almost exclusively in the vulvovaginal region and which has a wide differential diagnosis. It has previously been suggested that the nuclear transcription factor HMGA2 is a useful marker of AA, although the number of studies is limited. We investigated HMGA2 immunoreactivity in a large series (n=284) of vulvovaginal mesenchymal lesions. HMGA2 nuclear staining was classified as diffuse (≥50%), focal (<50%), or negative. Angiomyofibroblastoma is a rare mesenchymal benign tumor that frequently occurs in young- to middle-aged women, arising from the genital tract. There are many overlapping radiological and immunohistochemical features with other stromal cell lesions, making the diagnosis difficult. We report here a case of a 29-year-old woman admitted for a pelvic mass, in whom, the histopathological and immunohistochemical studies led to the diagnosis of angiomyofibroblastoma.

Male genital tract angiomyofibroblastoma (AMF) is a rare benign tumour, with a total of 34 cases reported in literature. We are presenting a case of AMF of the glans penis in a 68-year-old man who presented with a progressively increasing in size large lesion located on the tip of his penis. Following routine investigations, the lesion was surgically excised with no adjuvant treatment, the patient was followed-up for 5 years with no evidence of local, nodal or distant recurrence. As AMF of the glans penis is extremely rare, there is not enough literature to support management guide lines, but it appears that AMF responds very well to complete surgical excision; occasional cases of recurrence have been previously reported, so a long-term follow-up is advised...

Superficial angiomyofibroblastomas are mesenchymal tumours that occur in the genital tract and are well described pathologically. This case report reviews the imaging appearances and highlights the MRI findings, which have not been previously described. We describe the occurrence of this lesion in a vaginal cyst, which to the authors' knowledge, has also not been previously described. The histological findings are also presented here.

Paratesticular tumors account for 5% of tumors originating in the scrotum. Angiomyofibroblastoma-like tumor is a rare benign mesenchymal soft tissue tumor found in the genital region in both sexes and is even rarer in the spermatic cord. The origin of this tumor remains the object of speculation, but it is believed to originate from perivascular mesenchymal cells. The purpose of the present case report is to expand knowledge on the histology of this rare paratesticular tumor.
