Late degeneration is a feared complication of homograft aortic repair. As homograft use is usually associated with graft infection, a second open repair of a complication may be associated to significant mortality. Since 2013 it is mandatory for all centers in the Netherlands to register outcomes of all aortic aneurysm repairs in the Dutch Surgical Aneurysm Audit (DSAA). Analysis of DSAA data shows a reduction in mortality and complications after endovascular and open surgery for infrarenal abdominal aortic aneurysm (AAA) from 2015-2019. A late complication of aortic dissection is the dilatation of the weakened aortic wall. The only urgent endovascular alternative to high-risk open surgery in the treatment of postdissection aneurysms is branched endovascular aortic repair (BEVAR), which is extremely difficult due to the intimal membrane in the aorta lumen and the compressed true lumen.
