Lyme disease transmission to humans by Ixodes ticks is thought to require at least 36-48 h of tick attachment. We describe 3 cases in which transmission of Borrelia burgdorferi, the spirochetal agent of Lyme disease, appears to have occurred in less than 24 h based on the degree of tick engorgement, clinical signs of acute infection, and immunologic evidence of acute Lyme disease. Health care providers and individuals exposed to ticks should be aware that transmission of Lyme disease may occur more rapidly than animal models suggest...

When these nutritional supplements are infused during dialysis, losses are increased, but a net positive balance can be achieved if the infusion rate is high enough. Nocturnal lagophthalmos producing symptoms of corneal exposure is not uncommon. We diagnosed 40 patients with the condition over a six-month period. In 12 (30%), alcohol intoxication preceded the symptoms. Other factors included the use of hynotics, cosmetic blepharoplasty, botulinum toxin injection for blepharospasm, facial palsy, dysthyroid eye disease and cicatrising skin disease of the lids. In the largest group (17 patients, 42%), no cause could be found, but five of these had either first-degree relatives who slept with their eyes open, or had been observed with nocturnal lagophthalmos...

S-Adenosylhomocysteine (SAH) (0.1 to 30 mg/kg/i.p.) increased 3H-norepinephrine synthesis from 3H-tyrosine in the rat brain stem and mid brain, but did not alter its tissular level. It decreased both the serotonin level and the formation of 3H-serotonin from 3H-tryptophan and increased the formation of 3H-5hydroxyindolacetic acid. Its effects began 1 hr after administration and was still present 6 hr later. It did not alter dopamine metabolism. The present two-phase study was designed to determine the optimum hynotic dose of triazolam and to assess any evidence of acute tolerance. Subjective and objective data were used to assess hypnotic efficacy. In the dose-titration phase, the dose of triazolam was increased in 0-5 mg increments to the highest level tolerated by each individual patient. Maximum tolerable doses ranged between 0-5 and 3-0 mg; however, all subjects experienced maximum therapeutic effect at 1-5 mg triazolam. During the 4-night acute tolerance portion of the study no clear evidence of tolerance was found...
