A 58-year-old woman presented with an unexpected episode of vomiting and an acute abdomen. The patient mentioned a vague history of decreased appetite dyspepsia and constipation. Abdominal X-Ray revealed gas under diaphragm. An Open laparotomy was carried out to evacuate the free gas trapped under the diaphragm. The condition is almost always associated with perforation of abdominal viscera and accumulation of air during surgical or gynaecological procedures or peritoneal dialysis. In the reported case, laparotomy revealed no sign of perforation in GIT, uterine fundus or fistulas and nor did the have patient have any history of surgical or gynaecological procedures...

Pneumothorax associated with a pneumoperitonium in laparoscopic surgery is rare but can cause life-threatening complications. A 62-year-old man was scheduled for a laparoscopy-assisted Billroth-I gastrectomy under general anesthesia. Approximately 70 minutes after insufflating carbon dioxide into the intraabdominal cavity at a pressure of 12 mmHg, the peak inspiratory pressure increased, while the oxygen saturation decreased. The pneumothorax of the left lung was evident on the intraoperative chest radiograph...

Gastric perforation is a rare abdominal catastrophe which associated with high mortality in newborn infants. From June 1978 to July 1991, twelve cases of neonatal gastric perforation presented at Chang Gung Memorial Hospital. Male to female ratio was 9:3. The most common presenting signs were abdominal distension (100%), feeding intolerance (92%), respiratory distress (67%) and poor activity (58%). All cases had significant symptoms between two and five days of age. All of the abdominal plain film showed pneumoperitonium...
