The European Society for Gastrointestinal Endoscopy (ESGE) defines "difficult biliary cannulation" by the presence of one or more of the following: more than 5 contacts with the papilla, more than 5 minutes attempting to cannulate, or inadvertent cannulation of the pancreatic duct in 2 or more times (5-5-2 criteria), recommending these cut-off points to perform advanced cannulation techniques in order to reduce the rate of post-ERCP adverse events. Our objective was to evaluate the performance of the 5-5-2 criteria and their association with post-ERCP complications in a reference hospital in Peru...

INTRODUCTION: Bile duct stones constitutes 20% of symptomatic gallbladder disease and difficult stones are one third of this. Endoscopic papillary large balloon dilation (EPLBD) with or without prior endoscopic sphincterotomy (EST) produces excellent outcomes for the treatment of complex or difficult common bile duct (CBD) stones, alone or combined with previous therapeutic options as mechanical lithotripsy (ML) and temporary or definitive stenting. OBJECTIVES: Show success rate, complications and adverse events...

HISTORY AND CLINICAL FINDINGS: A 55-year-old man suffered from severe acute abdominal pain. 10 years previously he had been diagnosed with acute pancreatitis. On palpation, there was pronounced abdominal tenderness and guarding. INVESTIGATIONS: Emergency CT revealed signs of intra- and extrahepatic cholestasis and biliar sludge; serum-lipase was increased. TREATMENT AND COURSE: Acute biliary pancreatitis was diagnosed. After admission the patient's condition deteriorated; acute renal failure and respiratory insufficiency developed...

Paliative surgery in carcinoma of the head of the pancreas have thoroughly task to carry out decompression of biliar tract, remove duodenal impassability and reduce the pain. Numerous variants for decompression of biliar tract were recommended, but stil now there is no agreement about optimal approach. As method of choise for decompression of biliar tract in patients with cancer our first alternative is making safeless, with little traumatic and easy performance anastomosis. Duodenostasis in patients with carcinoma of the head of the pancreas was determinated clinicly in 40-60% of casees...

The most generally accepted therapy of choledochal cyst is cystectomy and biliar derivation by laparotomy. Last years, endoscopic papilotomy by ERCP has been a valuable therapeutic alternative, no only a diagnostic method. In this study, we reviewed five pediatric patients operated in our Deparment in last five years for choledochal cyst. The initial therapy was laparotomy (n=4) and endoscopic papilotomy by ERCP (n=1) This one was made in other Hospital. Follow-up has been between one and five years. All patients are living...

BACKGROUND: The prognosis of bilio-pancreatic cancer (pancreas, bile duct and gallbladder) is poor due to the fact of late diagnosis. The only curative treatment for such tumors is surgery. The 5-year survival rate is still below 5% and less than one-third of patients suffering from such carcinomas are resectable at the time of diagnosis. Although specific tumor markers do exist, to date there is no screening marker for these diseases. MATERIALS AND METHODS: Gallbladder bile of 44 consecutive cholecystectomized patients were prospectively analyzed for TAP concentrations...

COMPARATIVE STUDY

The etiopathogeny and clinical aspects of chronic pancreatitis were studied in 80 patients. From the etiological point of view it was observed that 75 out of the 80 patients (93,75%) had had previous intense alcohol ingestion (mean 239.95 +/- 158.34 g of pure ethanol/day) during an average period of 12.5 years. A study of their diet showed that these patients ate significantly more protein than the controls (p less than 0.001). In 2 of the remaining 5 patients, the familiar form of the disease was characterized; in 1 there was a previous history of biliar disease and no etiological factor could be detected...
