BACKGROUND: Little is known about the epidemiology of pediatric autoimmune blistering disorders (PAIBD). OBJECTIVE: We sought to determine the inpatient burden and comorbidities of PAIBD. METHODS: We analyzed data from the Nationwide Inpatient Sample from 2002 to 2012, which contained a representative 20% sample of all US hospitalizations. Methotrexate has been widely used for many years in the treatment of a variety of diseases. Acute pneumonitis and bone marrow suppression are very serious side effects in methotrexate treatment. A 48-year-old man with end-stage renal disease undergoing chronic hemodialysis developed combined acute pneumonitis and pancytopenia after a cumulative dose of 20 mg methotrexate for bullous pemphigoid. A 56-year-old male visited our hospital complaining of irregular-shaped atrophic erythematous plaques with blood crust, erosion, pigmentation, and depigmentation, localized on his forehead, bilateral cheeks, and ear lobes, for over one year. He has been receiving peritoneal dialysis and furosemide administration due to chronic renal failure for four years. We report a 78-year-old woman on hemodialysis who presented with refractory multiple pruritic vesicles and bullae on her trunk and extremities for 2 months. Histopathologic examination of skin biopsy specimen showed subepidermal bullae with many amyloid deposits in the papillary dermis. Intravenous immunoglobulins (IgIV) are therapeutic preparations of polyclonal human IgG, obtained from a pool of plasma from more than 1000 healthy donors. This article discusses only some of the recognized indications (Group I) and some of those currently under evaluation (Group II) for the treatment of autoimmune and systemic inflammatory diseases. Perspectives for IgIV use are approached through three diseases, each providing a different model of immune system modulation: ocular cicatricial pemphigoid, where an autoimmune reaction is modulated; alloimmunization of chronic hemodialysis patients, where alloreactivity can be modulated; and streptococcal toxic shock, where anti-infectious immunity may be modulated...

Two cases of bullous pemphigoid exclusively limited to the skin overlying a hemodialysis fistula in patients with chronic renal failure are reported. This is an unusual association, but should be taken into account in the differential diagnosis of bullous disorders among patients on hemodialysis. We hypothesize about the pathogenesis of this process.

We describe a case of bullous pemphigoid (BP) in a patient with chronic renal failure maintained on hemodialysis. We diagnosed BP by histopathological and immunofluorescence studies. The relationship between BP and chronic renal failure and/or hemodialysis is not clear, but we believe that immune disarrangement due to chronic renal failure and/or hemodialysis may have influenced the pathogenesis of BP in our case.
