INTRODUCTION: Given the great clinical relevance of the cerebrovascular disease, the incidence, nosology, vascular risk factors and factors predicting short and medium-term survival after stroke were evaluated in Lleida (Spain). PATIENTS AND METHODS: Five hundred forty-five consecutive patients with an acute stroke admitted to the Hospital Universitario Arnau de Vilanova during the period 1996-1997 were evaluated. A descriptive epidemiological study and a multivariate logistic regression analysis of predictive factors of mortality at 1-month and 1-year after the stroke were made...

Reactive airways disease in children is increasing in many countries around the world. The clinical diagnosis of asthma or reactive airways disease includes a variable airflow and an increased sensitivity in the airways. This condition can develop after an augmented reaction to a specific agent (allergen) and may cause a life-threatening situation within a very short period of exposure. It can also develop after a long-term exposure to irritating agents that cause an inflammation in the airways in the absence of an allergen...

The hantavirus is known to cause hemorraghic fever with renal syndrome (HFRS), which is widely spread in Europe and Asia. Several reports have shown an association of hantavirus antibody titers and the occurrence of renal dysfunction. From these observations, it appears that the virus is widely distributed, and different strains prevail in various areas. In the present work we studied 81 patients with end-stage renal-failure under hemodialysis (HD) treatment, 55 with mild to moderate renal failure, and 50 healthy subjects for the presence of antibodies to Hantaan and Puumala viruses...

We report a case of an oligodendroglioma associated with a cavernous angioma. The patient, a 20-year-old man with an 8-year history of epileptic seizures, presented an acute onset of headache, nausea, and vomiting. Computerized tomographic scan and angiogram revealed multiple vascular malformations in both hemispheres including a cystic hemorraghic lesion with a perifocal edema attributing to a mass effect. Repeated puncture of the cyst failed to give improvement of elevated intracranial pressure. At surgery, the cyst and the underlying lesion were excised and found to be oligodendrocyte-rich tissue with malformed vascular tissue...

Until recently only few cases have been described of acute infective endocarditis with E. Coli limited to a normal native mitral valve. Furthermore, mechanisms of so called abcess formation and rupture are still uncompletely understood. We report the case of an E. Coli endocarditis developing a rapidly progressive pseudoaneurysm of the mitral annulus. At necropsy diffuse infectious tissue weakening with pseudoaneurysm formation of the mitral ring and dissection into an hemorraghic pericard were seen. The authors further discuss the changing pattern of infectious agents causing acute infective endocarditis of the native mitral valve, transesophageal echocardiographic characteristics of paravalvular cavities and insights in mechanisms of pseudoaneurysm formation and dissection from clinicopathological findings...

Severe acute pancreatitis is morphologically characterized by an extensive and prolonged pancreatic and retroperitoneal inflammation with surimposed patchy or generalized areas of necrosis and hemorraghe in the pancreas and surrounding tissues. Clinical hallmarks include the early development of remote organ dysfunctions, notably cardiorespiratory failure and the late emergence of local complications (hemorraghe, acute pseudocyst and, most importantly, infection). The severity of the attack and the outcome of the patient are critically dependent on the presence and the extent of regional necrosis and are closely related to the casual bacterial contamination of these devitalized areas...

Neonatal anoxia induces a remarkable rise in brain bilirubin levels in the rat. The percentage of rats affected and the bilirubin levels reached are higher between the 3rd and 6th day after the anoxic episode. The accumulation of bilirubin in the brain could reflect the incidence of intracerebral hemorraghes (of different size and localization) caused by anoxia, and can plan an important role in the ethiology of the bilirubin encephalopathy and the neurological damage due to perinatal asphyxia.

Metalloproteinase from the venom of Bothrops asper (proteinase G) is a glycoprotein with 1% neutral hexose and 3.5 moles of sialic acid per mole of protein. It hydrolyses a number of protein substrates such as casein, hemoglobin, gelatin and fibrinogen, whose alpha chain is degraded preferentially. The pH optimum of hydrolysis of casein is approximately 9.5. The protease is devoid of hemorraghic, esterolytic and amidolytic activities. The proteolytic activity of the enzyme increases by about 20% in the presence of 0...

This article reviews arterial embolization of epistaxis and juvenile nasopharyngeal angiofibromas. A protocol for the complete and rapid exploration of epistaxis is suggested that is based upon which arteries providing blood to the nasal fossa may be responsible for the bleeding and the most optimal way for their demonstration and evaluation. The relevant anatomy of the major arteries, their branches, and important anastomoses are described. The goals of embolization are to control severe or recurrent epistaxis, prevent recurrence if possible, and avoid occlusion of any vessels not responsible for the hemorrhage...

Glanzmann's thrombasthenia begins in infancy. The most important clinical signs are cutaneous and mucous hemorraghes (purpura, epistaxis, bleeding gums, menorraghia after puberty). The disease proceedes by alternate periods of activity and remission, the frequency and severity of which decrease with age. Death occurred during any of the attachs. Our 10 cases (7 girls, 3 boys) were in 5 "Manouche" gypsies sibships, interrelated by consanguineous marriages. Genetic studies demonstrated that the disease was inherited in the autosomal recessive pattern...

Examining a case of death due to a subarachnoidal hemorraghe of a ruptured cerebral aneurysm the approximate time of death could only be determined by a considerable difference between alcohol contents of urine and blood. Since the urinary bladder was filled and a continuous diuresis until the moment of death could be assumed it was possible to calculate the average period of time during which the stated concentration of urine alcohol was formed, and to estimate the survival time.

Although interest in coagulation has usually centered about the various hemorraghic disorders, it is hyperactivity of the hemostatic system with its thrombotic complications which has a far greater clinical impact. The hypercoagulability states are a vague group of disorders not well defined by the laboratory. They fall into two distinct groups: (1) conditions promoting venous thrombosis by activating the coagulation mechanism and (2) conditions promoting arterial thrombosis by platelet plug formation. Dealing with both of these groups separately the various disorders associated with a hypercoagulable state are discussed as well as their pathophysiologic basis; Special emphasis is placed on the laboratory evaluation of these disorders...
