In this manuscript, we describe and discuss a case of pneumo-membranous labyrinth, with air bubbles extending also to the endolymphatic sac, resulting in anacusis, following hyperbaric oxygen therapy for sudden sensorineural hearing loss. In this short manuscript, we discuss the role and present the practical aspects of a guidewire inserted distally in the ipsilateral arm in facilitating venous localization and access in the context of cardiac rhythm management device implantation, reducing the risks for complications, such as pneumo- and hemothorax. We believe that this notion may encourage practicing device-implanting physicians to keep this approach in mind and as a fallback strategy during their cases.
