OBJECTIVES: To identify pre-operative patient/facility factors associated with post-operative and total episode-related costs using renal colic as a model surgical condition to improve value-based payment models. METHODS: Using state Healthcare Cost and Utilization Project data, we performed a retrospective cohort study examining peri-operative costs for individuals presenting to an emergency department for renal colic and who ultimately underwent definitive surgical management...

OBJECTIVES: To establish the patient-specific cost and time savings associated with telemedicine with the secondary environmental benefits of virtual visits within a tertiary referral center sub-specialty urology clinic. METHODS: An electronic health record query was made of all urology telehealth visits that have occurred between October 4, 2020 and October 10, 2020 at a single academic center. We evaluated the cost of travel for an in-person visit based on zip code data...

INTRODUCTION: We conducted a population-based analysis of Fournier's gangrene to compare risk factors and mortality with perineal cellulitis. METHODS: We analyzed National Inpatient Sample data (2016-2020) to identify Fournier's gangrene and perineal cellulitis cases. Demographic, comorbidity, and procedural data were extracted. Logistic models assessed risk factors for Fournier's gangrene diagnosis and mortality. RESULTS: A total of 73,472 cellulitis and 9,326 Fournier's gangrene cases were identified corresponding to 74,905 (range 63,050-79,165) and 9,115 (range 7,925-11,080) median yearly weighted cases, respectively...

INTRODUCTION: Patient-centered communication after surgery can enhance patient satisfaction and reduce unplanned clinical contact. However, patients information needs following kidney stone surgery are not well-understood, limiting quality improvement efforts. We aimed to characterize patient-reported needs in and preferences for postoperative communication following kidney stone surgery. METHODS: Patients undergoing common stone procedures were surveyed about the content, volume, and satisfaction with the communication they received after surgery...

INTRODUCTION: Prior work notes the AUA in-service exam (ISE) percentile ranking of chief residents correlates with qualifying exam (QE) performance. We present a 5-year analysis of resident performance on the ISE and subsequent QE to determine if earlier time points in training may identify those needing additional educational support. METHODS: Participant ISE scores over a 5-year period from 2014 to 2018 and subsequent QE scores in 2019 were recorded. Pearson's correlation coefficient measured the association between percentage questions correct for each ISE year and QE...

INTRODUCTION: How renal mass biopsy (RMB) impacts patient management with T1 renal masses (T1RM) is unclear. We explore the association between RMB and utilization of active surveillance (AS), nephron-sparing interventions (NSI), and radical nephrectomy (RN). METHODS: Data were analyzed retrospectively using the MUSIC-KIDNEY registry. Treatment received was analyzed using a fitted mixed-effects multinomial logistic-regression model. RESULTS: Of 4062 patients, 19...

PURPOSE: Both direct and indirect costs contribute to financial toxicity in prostate cancer. Indirect costs are difficult to assess and quantify and therefore remain understudied. We sought to describe the indirect costs of prostate cancer across risk groups as well as identify any associated sociodemographic factors. MATERIALS AND METHODS: Patients with localized prostate cancer were surveyed regarding their indirect costs at baseline and 3, 6, 12, and 24-month follow-ups...

PURPOSE: We sought to implement a multi-pronged behavioral intervention to reduce and tailor antibiotic use for two common urologic outpatient procedures. MATERIALS AND METHODS: This study was a non-blinded intervention study that consisted of a pre-intervention phase (11/2018-1/2019), an intervention phase (1/2020-12/2020) in which a multi-pronged behavioral intervention was implemented, and a post-intervention phase (1/2021-3/2021). We examined antibiotic use for cystoscopy and transrectal prostate biopsy at three separate urologic outpatient clinics...

INTRODUCTION: Placing ureteral stents at the uretero-ileal anastomosis for radical cystectomy with ileal conduit (RCIC) has long been common practice. Recently, some providers have begun omitting stents. We sought to investigate differences in perioperative and 30-day outcomes between patients who underwent radical cystectomy with ileal conduit (RCIC) with and without stents placed at the uretero-ileal anastomosis. METHODS: We identified RCICs performed between 2019 to 2021 in the National Surgical Quality Improvement Program database and corresponding Cystectomy-Targeted Participant Use File...

INTRODUCTION: Collagenase clostridium histolyticum (CCH) therapy for Peyronie's disease (PD) yields satisfaction rates of roughly 50% to 67% within 1 year of treatment completion, but little is known about long-term patient satisfaction. Our study aimed to identify clinical predictors of long-term satisfaction with CCH for PD and the impact of its side effect profile. METHODS: The Treatment Satisfaction Questionnaire for Medication (TSQM) survey was distributed to patients who received CCH for PD at a high-volume men's health academic center from 2009 to 2022...

INTRODUCTION: We investigated the risk of UTIs and complex UTIs associated with SGLT2 (sodium-glucose cotransporter-2) inhibitors in men, emphasizing older men at higher risk for voiding dysfunction. METHODS: Utilizing a pharmacovigilance case-noncase design, we analyzed VigiBase reports from 1967 to 2022 among male patients. VigiBase is a comprehensive global database for drug safety. Disproportionality analysis, which compares the frequency of reported adverse events for specific drugs against other drugs, was conducted using reporting odds ratio (ROR) and empirical Bayes estimator (EBE)...
