Varicella zoster virus (VZV) infection may cause large or medium vessel vasculitis, including granulomatous arteritis of the nervous system and central nervous system vasculitis. However, small vessel vasculitis, such as cutaneous leukocytoclastic vasculitis (LCV) associated with localized cutaneous VZV infection, herpes zoster, is uncommon. This is a case of a large pedunculated lipofibroma on the left axilla in a 36-year-old woman. The lesion was excised completely with no recurrence after one year of follow-up. A new name for this rare entity is proposed to be more inclusive of all lesions of pedunculated lipofibroma and to differentiate it from another entity called fibrolipoma.

Granuloma faciale (GF) is a rare benign chronic inflammatory dermatosis often difficult to distinguish clinically from other diseases, both inflammatory and neoplastic. Dermoscopy can be a helpful diagnostic tool and indeed several dermoscopic criteria observed in GF have been described in literature.
