The literature on childhood tuberculous empyema (TE) is limited. The aim of this study was to examine the clinicopathological characteristics and outcome of paediatric TE and methods of prompt diagnosis and treatment. Rhizobium radiobacter is an aerobic, motile, oxidase-positive, non-spore-forming Gram-negative tumorigenic plant pathogen which rarely infects humans. A 46-day-old girl was admitted to hospital with a 10-day history of fever and cough. She had pneumonia and liver dysfunction owing to infection by R. radiobacter . Thiamine (vitamin B1 ) is available in common foods such as the outer husk of rice and is necessary for normal cardiovascular, neurological and metabolic processes. Thiamine deficiency is common in many parts of Asia and Africa, affecting up to a third or more of children and women of child-bearing age. The diagnosis is based on clinical suspicion, especially when noting heart failure in infants, encephalopathy in patients of any age, and peripheral neuropathy in older children and adults. Tuberculosis-associated haemophagocytic lymphohistiocytosis (HLH) is rare in paediatrics and can be fatal if not recognised and treated on time. A 3-month-old infant with tuberculosis and HLH is described.
