STUDY OBJECTIVES: Treatment-emergent central sleep apnea (TECSA) describes the appearance or persistence of central sleep apnea while undergoing treatment for obstructive sleep apnea (OSA). TECSA is well-studied in continuous positive airway pressure (CPAP) therapy with an estimated prevalence of 8%. Based on few case reports, mandibular advancement devices (MAD) may also provoke TECSA. This study aims to gain insight into the prevalence of TECSA with MAD therapy. METHODS: This retrospective study includes a total of 129 patients with moderate to severe OSA who were treated with a custom-made titratable MAD...

STUDY OBJECTIVES: The treatment of obstructive sleep apnea is often impeded by intolerance of positive airway pressure therapy, which is frequently attributed to the inability to breathe through the nose. Providers caring for patients with sleep apnea need a working knowledge of nasal passage disease and available treatments to better manage this common comorbidity. METHODS: This review examines the literature connecting rhinosinusitis to adverse sleep and sleep apnea outcomes...

Growing evidence suggests that transgender individuals face a significant health disparity and are particularly vulnerable to sleep disorders. We present two patients who developed irregular sleep-wake rhythm disorder after gender reassignment and hormone replacement therapy. The growing interest in transgender health warrants further evaluation of the effects and frequency of all sleep disorders in this population. Efforts to address sleep disorders should consider assessing sleep disturbance in terms of sleep/wake patterns and schedules...

STUDY OBJECTIVES: Evaluate the quality of ChatGPT responses to questions on obstructive sleep apnea (OSA) for patient education. Assess how prompting the chatbot influences correctness, estimated grade level, and references of answers. METHODS: ChatGPT was queried four times with 24 identical questions. Queries differed by initial prompting: no prompting, patient-friendly prompting, physician-level prompting, and prompting for statistics/references. Answers were scored on a hierarchical scale: incorrect, partially correct, correct, correct with either statistic or referenced citation ("correct+"), or correct with both a statistic and citation ("perfect")...

STUDY OBJECTIVES: The relationship between time-limited eating (TLE) and sleep quality is a topic of growing interest in the field of chronobiology. Data in adult cohorts shows that TLE may improve sleep quality, but this has not been evaluated in adolescents. The aim of this secondary analysis was to (1) examine the impact of 8-hour TLE on sleep parameters in youth with obesity and (2) explore if there was any association between sleep patterns and glycemic profiles. METHODS: Adolescents with obesity were randomized into one of three groups: 8-h TLE (participants self-selected their eating window) + real-time continuous glucose monitor (CGM), 8-h TLE + blinded CGM, or a prolonged eating window...

Shrinking lung syndrome (SLS) is a rare complication of systemic lupus erythematosus (SLE); its impact on sleep-disordered breathing (SDB) is not well understood. We describe a case of a 36-year-old female with SLE experiencing SLS and frequent pain crises. We review manifestations of her disease during non-rapid eye movement (NREM) and rapid eye movement (REM) sleep. SLS with its restrictive physiology and associated diaphragmatic myopathy is expected to decrease minute ventilation during NREM and REM sleep...

STUDY OBJECTIVES: Child sexual abuse (CSA) is associated with negative consequences on both physical and mental health. It has been found to influence child and adolescent sleep, which is an important developmental factor, which also influences mental and physical health. However, the literature examining this topic is plagued by methodological limitations, such as small sample sizes and unvalidated measures. METHODS: Using the Pittsburgh Sleep Quality Index, the present study examined seven subjective sleep dimensions including, sleep latency, efficiency, duration, disturbances, subjective sleep quality, daytime dysfunction, and use of sleep medication, in a sample of 707 adolescent girls aged 14-18 years old...

Combination positive airway pressure (PAP) therapy and daytime transoral neuromuscular electrical stimulation (NMES) in the treatment of sleep apnea has not been reported in the literature. We present a case of a patient with poorly controlled sleep apnea despite bilevel PAP. Adjunctive therapy with daytime NMES resulted in dramatic reduction of the apnea-hypopnea index with significant improvement in patient's symptoms.

Sleep-related sexualized behaviors occur in the parasomnia known as sexsomnia, recognized as a variant of confusional arousals in the ICSD-3. These instinctive behaviors of a sexual nature emerge from deep NREM sleep and patients often present with distinguishing features within this sleep disorder category. There are often adverse psychosocial consequences, and not uncommonly medico-legal implications. While associations to psychiatric consequences from the sexsomnia have been demonstrated and efforts to further typify this condition have been made, sexsomnia remains incompletely characterized in the more than 200 published cases to date, with male predominance...
