Breast cancer incidence and mortality continue to increase in Africa. In Tanzania, breast cancer is the second leading cause of cancer death for women, and breast cancer incidence and mortality are projected to increase by 80% by 2030. Education gaps among healthcare workers, delayed presentation, limited screening, and low health literacy all pose significant challenges to providing optimal breast cancer care. Considering these factors, it is imperative to train expert breast radiologists. We present a collaborative breast radiology training program in Tanzania aimed at building local capacity to address national breast cancer challenges...

The colonial origins and power imbalances between western high-income countries (HICs) and low and middle-income countries (LMICs) are barriers to self-reliance and sustained structural improvements to healthcare systems. Radiologists working in global health (Global Radiologists) are tasked with improving the state of imaging in LMICs while mitigating the effects of colonial structures and processes. To accomplish this, we need to be aware of factors such as colonialism, neocolonialism, parachute research, and brain drain that contribute to global health inequities...

The migration of healthcare workers on a global scale has emerged as a complex and critical issue, profoundly impacting the dynamics of healthcare systems worldwide. This paper delves into the multifaceted drivers behind the unprecedented migration of healthcare professionals, seeking to illuminate the interconnected factors that propel this phenomenon. Our investigation encompasses economic, social, and professional dimensions, acknowledging the intricate interplay of factors influencing the decision of healthcare professionals to migrate...

OBJECTIVE: The Ugandan Ministry of Health adopted the Breast Imaging-Reporting and Data System (BI-RADS) as standard of care in 2016. We performed a medical audit of breast ultrasound practices at four tertiary-level hospitals to assess interpretive performance. We also determined the effect of a low-cost navigation program linking breast imaging and pathology, on percent of patients completing diagnostic care. METHODS: We retrieved 966 consecutive diagnostic breast ultrasound reports, with complete data, performed on women aged >18 years, presenting with symptoms of breast cancer between 2018-2020 from participating hospitals...

OBJECTIVE: To quantify the relative importance of demographic, contextual, socioeconomic, and nodule-related factors that influence patient adherence to incidental pulmonary nodules (IPNs) follow-up visits and evaluate the predictive performance of machine learning models utilizing these features. METHODS: We curated a 1,610-subject patient dataset from EHR consisting of thirteen clinical and socioeconomic predictors and IPN follow-up adherence status (timely/delayed/never) as the outcome...

BACKGROUND: Coronary artery disease (CAD) is the leading cause of death in the United States. At risk asymptomatic adults are eligible for screening with EKG-gated coronary artery calcium (CAC) CT, which aids in risk stratification and management decision-making. Incidental CAC (iCAC) is easily quantified on chest CT in patients imaged for noncardiac indications; however, radiologists do not routinely report the finding. OBJECTIVE: To determine the clinical significance of CAC identified incidentally on routine chest CT performed for non-cardiac indications...

Diagnostic errors in radiology often result from incomplete visual assessments attributed to insufficient training in search patterns. Radiologists lack consistent feedback, relying on ad-hoc strategies, leading to suboptimal patterns and potential false negatives. This study used eye tracking to analyze search patterns, quantify performance metrics, and evaluate the impact of an automated continuous improvement feedback-driven framework on detection accuracy. In a controlled trial with ten residents, the intervention group (received feedback) demonstrated a significant 38...

OBJECTIVE: To assess individual- and neighborhood-level sociodemographic factors associating with providers' ordering of non-pharmacologic treatments for patients with low back pain (LBP), specifically physical therapy (PT), image-guided interventions (IGI), and lumbar surgery (LS). METHODS: Our cohort included all patients diagnosed with LBP from 2000-2017 in a state-wide database of all hospitals and ambulatory surgical facilities within Utah. We compared sociodemographic and clinical characteristics of 1) LBP patients who received any treatment with those who received none, and 2) LBP patients who received invasive LBP treatments with those who only received non-invasive LBP treatments using the Student's t test, Wilcoxon rank sum tests and Pearson's chi-squared tests as applicable and two separate multivariate logistic regression models: 1) to determine whether sociodemographic characteristics were risk factors for receiving any LBP treatments and 2) risk factors for receiving invasive LBP treatments...

PURPOSE: To determine medical students' views of various aspects of a career in radiation oncology (RO), in order to identify areas that may benefit from reform, and guide initiatives to stimulate broader and more diverse student interest in the specialty. METHODS AND MATERIALS: An electronic survey was sent to student oncology interest group members at 7 U.S. medical schools. The survey asked students to rate 19 aspects of RO on a 5-point bipolar Likert-type scale...
