An 82-year-old man presented with a growth on his scalp for 7 months. It was increasing in size despite treatment with a variety of shampoos and systemic plus topical antibiotics. It was pruritic with a burning sensation. Lately, the patient had a body weight loss of 35 pounds. Extensive work-ups for malignancy included computerized axial tomography (CAT) and examinations of the gastrointestinal and respiratory tracts; however, no etiology for this weight loss was established. This was not thought to be directly related to the scalp lesion...

A 69-year-old patient with no personal or family history of ichthyosis consulted our dermatology department for diffused cutaneous xerosis with intense pruritus evolving for 3 weeks. Physical examination revealed diffused ichthyosis of large polygonal fine scales on the skin without erythema (Figure 1). The lesions spared the face. Examination of the mucous membranes, hair, and nails revealed no abnormalities. There was no fever or adenomegaly. A skin biopsy revealed an orthokeratotic hyperkeratosis with thinning of granular layer (Figure 2)...

A 36-year-old woman presented to the emergency department with a 1-day history of purulent perianal and vulvar discharge. She had a 25-year history of Crohn's disease (CD), and 13 years prior had received a total colectomy with end ileostomy. She had vulvar biopsies 5 years prior to presentation, demonstrating noncaseating granulomas consistent with metastatic Crohn's disease (MCD). Throughout the course of her disease, she had a failed treatment with adalimumab, certolizumab, methotrexate, and 6-mercaptopurine...

YCANTHTM (cantharidin) topical solution has been approved recently for the treatment of molluscum contagiosum (MC) in children (aged ≥2 years) and adults. It works by activating serine proteases that lead to blistering and inflammation, promoting shedding of infected cells and viral clearance. In two phase-3, randomized, double-blind, vehicle-controlled trials of similar design, VP-102 (a drug-device combination, containing cantharidin 0.7% w/v and inactive ingredients, such as gentian violet, acetone, and denatonium benzoate, administered with an applicator) was investigated for the treatment of MC...

ZORYVETM (roflumilast) cream is a topical phosphodiesterase-4 (PDE-4) inhibitor that has been recently approved for the treatment of plaque psoriasis. It is also indicated for use in intertriginous areas. Roflumilast, the active ingredient, inhibits PDE-4, leading to the suppression of pro-inflammatory immune responses in psoriatic lesions. Two phase 3 clinical trials have demonstrated the efficacy of once daily application of roflumilast to treat plaque psoriasis in patients aged 12 years and older. At week 8, an investigator's global assessment score of 0 or 1 with a grade 2 improvement from baseline, the primary efficacy end point, was observed in 39...

Human immunodeficiency virus (HIV) is a potentially life-threatening single-stranded RNA lentivirus that attacks the body's immune system by infecting CD4 T lymphocytes. If untreated, HIV infection can progress to acquired immunodeficiency syndrome (AIDS).1.2 With the US prevalence of around 12.6 per 100,000 individuals, HIV positivity remains undiagnosed among much of the population.3 The most common presentation of HIV includes flu-like manifestations, such as fever, malaise, diarrhea, weight loss, headache, and muscle aches; however, it is also known to present as other diseases, including psoriasis...

A 45-year-old woman presented to the outpatient dermatology clinic during the COVID-19 pandemic with a painless green discoloration of several fingernails present for 1 week (Figure 1). The patient had no significant past medical history and was not taking any medication. No other findings were noted. There was no history of nail trauma or past fungal infection of the nail. The patient admitted that she had frequent hand washing, given the current pandemic, and never used protective gloves in everyday activities...

Xeroderma pigmentosum (XP) is a rare autosomal recessive disease; relatively mild XP patients are sometimes designated as having pigmented xerodermoid or xerodermoid pigmentosum (XP-V), a variant of XP. It is commonly associated with many long-standing skin conditions and tumors, including malignancies, management of which is necessary to prevent the progress of the disease. The objective of the study was to evaluate an innovative therapeutic treatment, beyond surgery, surgical excision, cryotherapy, electrocautery and curettage, or Mohs surgery, for the management of skin tumors in XP...

Important components of a medical journal include its readers, authors, editor, and owner. Editor is the individual to whom the journal is branded. The editor determines the journal's published content and establishes its caliber. The success of a journal depends on the general and specific responsibilities of the editor toward its readers, authors, and owner. For a journal to maintain its stature of excellence, the expectations of the editor-including editorial independence-must be preserved; therefore, in the best interest of the journal, the owner must provide unequivocal support to the editor...

In dermatology, the role of dietary modifications as a means to manage or prevent skin disorders has recently gained special attention among patients and physicians. This is especially true for the currently popular ketogenic diet (KD), which comprises low carbohydrate, high fat, and adequate amount of protein. Recent evidence from basic science research, small clinical trials, population studies, and reports has presented promising potential role of KD as a supplementary or adjuvant treatment in different cutaneous disorders mainly due to its anti-inflammatory properties...
