We describe the clinic, image, and histopathologic features of a well differentiated neuroendocrine carcinoma (carcinoid tumour) metastatic to choroid and ciliary body in a 52-year-old Mexican Mestizo man. The ophthalmologic examination showed an inferior choroidal mass accompanied by exudative retinal detachment. Ultrasound B-Scan study revealed a diffuse thickened choroid with overlying serous retinal detachment, ultrasound A-Scan revealed a high internal reflectivity solid lesion. Ultrasound biomicroscopy (UBM) evidenced a dome shaped ciliary body mass, presumptive diagnosis was uveal tract metastatic disease...

Munchausen's Syndrome is a factitious disorder in which patients manipulate physicians through misrepresented signs and symptoms. The authors describe an 18-year-old woman with self-induced inoculation of ant's particles inside her conjunctival fornices of both eyes. The crushed particles was confirmed to be crushed ant's particles. Munchausen Syndrome can be seen with ophthalmic manifestations and should be considered in the differential diagnoses when ocular abnormalities cannot be explained. A sympathetic and supportive approach is therefore required and these patients should be urgently referred to a psychiatrist...

Ring scotoma is an annular field defect centered on fixation. Age-related macular degeneration in the elderly and hydroxychloroquine toxicity in younger patients are usual causes of central ring scotoma. We report bilateral ring scotoma as the presentation of adult onset Stargardt's disease. Central ring scotoma has a precise localizing value, to a lesion involving macula.

Atheer Ali Alduaij, Dhoha Alhamad, Patrik Schatz
Central retinal vein occlusion (CRVO) is relatively rare in the pediatric age group. We present a case of CRVO as the first manifestation of diabetes mellitus in a ten-year-old girl. The associated macular edema was managed successfully with a single injection of Ranibizumab. Ophthalmologists should consider the possibility of diabetes mellitus in pediatric cases of CRVO.

We present a case of spontaneous in situ breakage of intracorneal ring segments (ICRSs) 8 years after their implantation in a patient with keratoconus. The patient presented to our clinic with a red and painful right eye that had not improved despite topical steroids and antibiotics. The decision was made to explant the broken ICRSs from the cornea and send them for laboratory analyses, by which manufacturer defects were excluded. It is noteworthy that corneal curvature re-steepening was observed 4 months after ICRS removal despite the performance of crosslinking (CXL) 1 year prior to ICRS implantation...

Molluscum contagiosum is a benign viral cutaneous infection. It typically presents as an asymptomatic centrally umbilicated nodule 3-5 mm in diameter. Susceptible patients are children and adults receiving immunosuppressive therapy. We report a case of an eyelid molluscum contagiosum in a 5-year-old boy with no risk factors and a 2-week history of a large localized ulcerating mass of the left upper eyelid. The mass was totally excised, and diagnosis of molluscum contagiosum was confirmed in the histopathology study...

Bilateral Idiopathic Orbital Inflammation Syndrome (IOIS) is rare in adults. Most commonly it affects the young adults and children. It is mostly unilateral condition. This is a case of 50 years old male coming with the complaints of painless proptosis on both the eyes for 1 year. The history of diplopia was also found. On examination, it was found that there was restriction of movements in Right eye superiorly. Any history of associated systemic illness was not found. Based on the clinical examination, laboratory reports and contrast enhanced computed tomography (CECT) the diagnosis of Bilateral IOIS was made...

This paper describes a novel approach to treat migrated orbital implants post socket surgery. Implant migration may hinder the final aesthetic outcome of a custom ocular prosthesis. Once an implant migrates within the orbit there tends to be fibrosis around the implant. This fibrosis does not allow for centeration of the implant during repeat surgery. Hence treatment of a migrated implant traditionally involves implant removal with dermis fat grafting. Dermis fat graft though an option, needs a second site surgery that may be unacceptable to many patients...

Saeed Alwadani
Cataract surgery is the most common surgery to face the ophthalmology training resident. To facilitate achieving surgical competency and reduce complication rates, wet laboratories and surgical simulators are used in surgical disciplines worldwide. We developed a simulator and wet-lab course that aims to build the microsurgery skills of trainees and improve safety during real surgical procedures. Herewith, we describe the standardized hands-on course that incorporates these tools for advanced training. Additionally, we review the literature on wet-lab and surgical simulators in ophthalmology, focusing on their importance in training centers...

Diabetic retinopathy (DR) is a retinal disease which is one of the most severe complications occuring due to diabetes mellitus and is a major cause of blindness. Patients who have diabetes mellitus for number of years develop characteristic group of lesions in the retina which leads to Diabetic retinopathy. It is a multifactorial condition occuring due to complex cellular interactions between biochemical and metabolic abnormalities taking place in all retinal cells. Considerable research efforts in the past 20 years have suggested that the microvasculature of the retina responds to hyperglycemia through a number of biochemical changes, which includes polyol pathway, protein kinase C activation, upregulation of advanced glycation end products formation and renin angiotensin system activation...

Setting: 2 centers (university-based practice and a private practice set up). Patients and methods: Prospective nonrandomized study in which 74 AT LISA TRI 839MP and 8 AT LISA TORIC TRI 939MP IOLs implanted bilaterally in 41 patients following either cataract extraction or refractive lensectomy, follow-up was done at 1st, 2nd and 3rd months to assess the visual and refractive outcomes...

Amal ElBendary, Amr H Elkannishy
Purpose: To compare the efficacy of diclofenac versus bevacizumab following single intravitreal injection in eyes with persistent diabetic macular edema. Methods: Fifty eyes with persistent DME were randomly allocated to intravitreal injection of 500 µg/0.1 ml of diclofenac ( N  = 20) or 1.25 mg/0.05 mL of bevacizumab ( N  = 20) or to non-treatment (10 eyes). Preoperative and postoperative visual acuity, central, paracentral macular thickness and intraocular pressure (IOP) were recorded and compared between the three groups up to 4 weeks...

Purpose: To compare the diagnostic ability of optical coherence tomography angiography (OCT-A) derived radial peripapillary capillary (RPC) measured capillary density (CD) and inside the optic nerve head (ONH) CD measurements to differentiate between the normal and primary open angle glaucoma (POAG) eyes. Methods: AngioVue disc OCT-A images were obtained and assessed in 83 eyes of POAG patients and 74 age matched healthy eyes. RPC CD was quantitatively measured in the peripapillary area within 3...

Purpose: To compare the efficacy and complications associated with higher viscosity (5000-centistoke) versus lower viscosity (1000-centistoke) silicone oil in traumatic retinal detachment surgery. Material and methods: Patients who underwent 23-gauge pars plana vitrectomy using 1000- or 5000-centistoke silicone oil to treat retinal detachment associated with traumatic globe injury were retrospectively analysed. Anatomical and visual outcomes were compared. Results: Forty-four eyes of 44 patients were included in the study, which included 22 eyes in each group...

Purpose: To monitor the outcomes of surgical correction of esotropia in order to improve preoperative counselling for patients and their families. Methods: A retrospective review audit of the medical charts at King Fahad Medical City. All patients treated surgically to correct esotropia, from January 2007 to December 2013.
