#1
P A Heasman
The science and practice of dentistry are underpinned by research and clinical developments that have been evaluated rigorously before being presented to the profession in peer-reviewed journals or at national and international meetings. The online platform that supports open access journals has been corrupted by the emergence of so-called 'predatory journals', which accept papers on payment of a significant publication fee. This practice has now extended to dental conferences and this article describes the author's personal views of predatory conferences...

N Karsondas, N Karsondas, P Blaylock
Two-year longitudinal dental foundation training (LDFT) programmes are available for dental graduates through a national recruitment process. These two-year schemes have been around for many years and enable foundation dentists to gain valuable experiences in general practice as well as the hospital and community services. However, the LDFT schemes have closed in Northern Ireland, Scotland and Wales in recent years, leaving the North East England General Professional Training (GPT) scheme and the Yorkshire and Humberside LDFT scheme as the only two LDFT/GPT schemes left...

National safety standards for invasive procedures (NatSSIPs), published by NHS England in 2015, recommend the creation and implementation of local safety standards for all invasive procedures. This includes procedures undertaken outside a hospital environment, such as surgical procedures undertaken by dentists. In order to implement a local safety standard for invasive procedures (LocSSIP) for oral surgery procedures at a large London teaching hospital, a clean sheet redesign of our service was carried out based on a bottom up model of transformation, using a 'diagnose, design and implement' strategy...

S Mickenautsch, V W Berger
It has been argued that the randomised controlled trial design is unsuitable for restorative dentistry and that cohort studies or the analysis of large observational datasets without randomisation is more suitable. This opinion article examines why randomisation in clinical trials is needed and why big observational data is not enough for clinical inference.

M A Bishop
The patient-dentist relationship is a delicate partnership that forms the core of dentistry. The attack on this partnership by the General Dental Council has led to gross miscarriages of justice and the emergence of defensive dentistry. This essay considers the role of a governing body, the misconduct of the GDC and the response of the profession to the injustices perpetrated. The system of NHS general dental practice is considered in terms of quality and humanity and found to be failing in quality and lacking in humanity...

It was a watershed moment when Archie Cochrane published his work on Effectiveness and efficiency: random reflections on health services which was considered the start of evidence-based healthcare as we now know it. From there onward, evidence-based dentistry (EBD) has developed and evolved and there has been little attention to the interlink between EBD and ethics. The aim of this paper is to explore some of the ethical basis of evidence-based approach to healthcare utilising Beauchamp and Childress' four principles...

P Jevon
Foreign-body airway obstruction (FBAO) (choking) is a life-threatening emergency. Each year in England and Wales, there are approximately 250 deaths caused by FBAO, 60% of which occur in the healthcare setting. Dental staff must be able to recognise and effectively treat FBAO. The aim of this article is to outline the recognition and treatment of choking in the dental practice.
