This clinical case shows the repercussions of acute exposure to cement dust in the respiratory tract and other mucous membranes. Following a cement dust pipe explosion, the patient endured a severe inhalation of dust. A combination of rigid bronchoscopy and a mechanical insufflation-exsufflation system was employed to remove cement debris from the airways. Respiratory physiotherapy sessions were implemented for effective secretion clearance, contributing to a successful short-term recovery. While this remains an isolated case, the unconventional techniques employed provide valuable insights for potential similar scenarios in the future...

OBJECTIVES: The optimal surgical strategy for primary spontaneous pneumothorax remains a matter of debate and variation in surgical practice is expected. This variation may influence clinical outcomes, such as postoperative complications and length of stay. This national population-based registry study provides an overview and extent of variability of current surgical practice and outcomes in the Netherlands. METHODS: To identify national patterns of care and between-hospital variability in the treatment of primary spontaneous pneumothorax, patients who underwent surgical pleurodesis and/or bullectomy between 2014- 2021, were identified from the Dutch Lung Cancer Audit-Surgery database...

OBJECTIVES: After staged reconstruction for hypoplastic left heart syndrome (HLHS), the neoaortic root tends to dilate, and the incidence of significant neoaortic valve insufficiency increases with time. This study aimed to evaluate the mid-term outcomes of the neoaortic root geometries and valve function after chimney reconstruction in the Norwood procedure. METHODS: Between 2013 and 2021, 20 consecutive patients who underwent chimney reconstruction during the Norwood procedure for HLHS and its variants in our institution were enrolled...

Endovascular aortic arch repair (Ar-TEVAR) with single-branch, off-the-shelf Nexus aortic arch system has shown promising early and 3-year outcomes. There is lack of data regarding long-term results of this specific device, particularly regarding patency of supra-aortic bypasses and stability of the connection between the two modules. This case report describes 6-year clinical and imaging follow-up of a 74-year old patient who underwent Ar-TEVAR with Nexus. Over six years, the patient experienced minor complications unrelated to the device...

OBJECTIVES: Depression has been associated with increased cardiovascular morbidity and mortality. This study aimed to determine whether self-reported preoperative depressive symptoms were associated with worse long-term survival in patients undergoing cardiac surgery. METHODS: This population-based, observational cohort study included patients who had undergone cardiac surgery at Karolinska University Hospital between 2013 and 2016. Self-reported data about depressive symptoms were collected using the Patient Health Questionnaire (PHQ-9) and other patient data were collected from the institutional surgical database and medical charts...

OBJECTIVES: Solitary fibrous tumours of the pleura (SFTP) are historically considered to be benign soft tissue neoplasms. However, a clinical relevant number of these neoplasms have malignant histological features. The objective of this study was to evaluate the percentage of SFTP presenting unfavorable clinical behaviour in order to predict negative long-term outcome. METHODS: A retrospective review of 74 patients treated at 4 hospitals between 1990 and 2013 was performed...

OBJECTIVES: The Ross procedure represents an excellent treatment option in younger patients with aortic stenosis but is limited by poor availability of homografts. In this study, we investigated the hydrodynamic performance of three different types of right ventricular outflow tract replacement with pericardium or synthetic material. METHODS: Three different types of valved conduits were constructed using pericardium and/or synthetic material (Group PEPE: pericardial cusps and pericardial conduit, Group PEPR: pericardial cusps and Dacron conduit, Group PRPR: expanded polytetrafluoroethylene cusps and Dacron conduit)...

OBJECTIVES: To evaluate the safety and feasibility of removing drainage tubes at larger size of air leak in patients with prolonged air leak (PAL) after pulmonary surgery. METHODS: Ninety-five patients who underwent pulmonary surgery with PAL in our center were enrolled in this randomized controlled, single-center, non-inferiority study. The drainage tube was clamped with a stable size of air leak observed over the last 6 hours, which was quantified by gas flow rate using the digital drainage system...

OBJECTIVES: The aim of this study was to assess the safety of early chest tube removal (CTR) protocol on the first postoperative day (POD1) of our enhanced recovery after surgery (ERAS) programme by comparing the risk of postoperative pneumothorax, pleural and pericardial effusion requiring intervention and hospital mortality. METHODS: All consecutive patients undergoing elective coronary revascularization and/or valve surgery between 2015 and 2021 were assessed in terms of their perioperative management pathways: conventional standard of care (control group) versus standardized systematic perioperative ERAS programme including an early CTR on POD1 (ERAS group)...

OBJECTIVES: Multi-organ failure is one of the leading causes of mortality after cardiac surgery for infective endocarditis (IE). Although the randomized evidence does not support the use of hemoadsorption during cardiac surgery for IE, observational studies suggest a beneficial effect in selected patient groups. S. aureus is the most common pathogen, and its presence is an independent mortality predictor. We aimed to analyze the effect of hemoadsorption in patients with IE caused by S...

OBJECTIVES: Compared to lung resections, airway procedures are relatively rare in thoracic surgery. Despite this, a growing number of dedicated airway centers has formed throughout Europe. These centers are characterized by a close interdisciplinary collaboration and they often act as supra-regional referring centers. To date, most evidence of airway surgery comes from retrospective, single-center analysis as there is a lack of large-scale, multi-institutional databases. METHODS: In 2018, an initiative was formed, which aimed to create an airway database within the framework of the ESTS database (AIR-ESTS)...

OBJECTIVE: To compare oncologic outcomes after segmentectomy with division of segmental bronchus, artery, and vein (complete anatomic segmentectomy) versus segmentectomy with division of < 3 segmental structures (incomplete anatomic segmentectomy). METHODS: We conducted a single-center, retrospective analysis of patients undergoing segmentectomy from 03/2005-05/2020. Operative reports were audited to classify procedures as complete or incomplete anatomic segmentectomy...

OBJECTIVES: The desirable lymph node count was determined to maximize the survival time expectancy according to the lymph node count among patients that have undergone oesophagectomy for oesophageal cancer. METHODS: The data of national Finnish population-based cohort including all patients that have undergone oesophagectomy due to oesophageal cancer 2002-2016 was used. Restricted mean survival time (RMST) estimates were determined by lymph node count and the desirable count was determined using locally estimated scatterplot smoothing regression...

OBJECTIVE: Heart transplantation for adult congenital heart disease is complicated and associated with challenging pretransplant support, long waiting, and high early post-transplant mortality. We explored if surgical and medical advances, and allocation system changes have affected outcomes. METHODS: From United Network for Organ Sharing database, adults with congenital heart disease listed for heart transplantation were queried. To explore practice and outcome trends, patients were divided into 4 eras (eras1-3: nearly 3 equal periods from 1992-2018, era-4: after 2018, corresponding with new allocation system)...

OBJECTIVES: Pulmonary resection in patients with severe emphysema may impact postoperative respiratory complications. Low attenuation areas evaluated using three-dimensional computed tomography to assess emphysematous changes are strongly associated with postoperative respiratory complications. Herein, we investigated the relationship between low attenuation area, the surgical procedure, and resected lung volume, which has not been explored in previous studies. METHODS: We retrospectively evaluated patients with primary or metastatic lung cancer who underwent surgical resection...
