PURPOSE: The aim of this study was to evaluate the clinical and radiological outcomes in the treatment of acute acromioclavicular joint (ACJ) separations using an all-endoscopic double cerclage endobutton technique compared to an arthroscopic-assisted single bundle endobutton technique. The secondary aim was to analyze the complication, failure and revision rate of both procedures. MATERIAL AND METHODS: All patients with acute ACJ separations Rockwood IIIB and V who were operated on using an all-endoscopic double-cerclage technique (2019-2022) or an arthroscopic-assisted single bundle technique (2017-2019) were included...

Diminished hip labrum size and tissue quality may be a predictor of poor patient outcomes when a nonaugmented primary labrum repair is performed. Labrum augmentation is an option for patients with hypoplastic or degenerative labral tissue. The optimal graft for augmentation has yet to be identified, and biomechanical research shows no difference in force to suction seal disruption between dermal allograft and iliotibial band allograft when used to augment the labrum. However, time zero biomechanical studies do not reflect biological ability of the graft to heal to surrounding structures, revascularization of the graft, durability of the graft, hip capsular status, and response to functional demands of the patient...

Bacterial contamination, as a result of suture contamination, may be associated with rotator cuff retear, or impaired tendon healing, following rotator cuff repair. This represents a potential new area of focus and intervention. Despite an array of rotator cuff repair techniques, from varying suture configurations and double row repairs to biologic adjuncts and patches, a substantial proportion of repairs do not heal or go on to retear. Decades of research have been dedicated to identifying risk factors for these failures and mitigating their influence...

PURPOSE: To (1) evaluate and synthesize the available literature related to platelet-rich plasma (PRP) treatment of knee pathologies and (2) provide recommendations to inform future research in the field. METHODS: PubMed, CINAHL, and Scopus databases were queried on October 6, 2023. All identified citations were collated and uploaded into Covidence for screening and data extraction. Studies were included if they were human studies published in English with adult cohorts that received PRP as a procedural injection or surgical augmentation for knee pathologies with patient-reported outcome measures (PROMs) and level of evidence I-IV...

Massive rotator cuff tears (MRCTs) are a challenge to treat with options from arthroscopic rotator cuff repair to reverse shoulder arthroplasty. Arthroscopic repair may still be an option even in the setting of advanced Goutallier changes and Hamada grades and regardless of healing. As the lifetime risk of revision for a 55 year old patient having an RSA was approximately 1 in 3, arthroscopic surgical management of MRCTs should not be discounted. An individualized approach that should consider the patient's radiographs (severity of arthritis), function (i...

Recent research shows that magnetic resonance arthrogram (MRA) offers better definition of certain shoulder pathology, especially labral tears, compared to magnetic resonance imaging (MRI). Conventional MRI accuracy and precision deteriorates after two weeks, and few health delivery environments achieve MRI within 14 days of a shoulder dislocation. Moreover, further loss of MRI accuracy is time dependent. While MRA should be considered as a first-line imaging study when evaluating shoulder instability more than two weeks after the patient's injury, one must also consider clinical value and cost-effectiveness...

In recent years, there has been a shift in healthcare away from the fee-for-service model to a value-based care model. Concomitantly, there have been changes in inflation-adjusted surgeon reimbursements for arthroscopic meniscectomies. Nationally, albeit at different rates, there has been a decrease in partial meniscectomy utilization. We feel that this is in part due to the change in healthcare economics and reimbursements as well as the growing evidence that partial meniscectomy may not be as efficacious in the treatment of degenerative meniscus tears as we once thought...
